Description

Coralan 5 mg Tablet is a prescription medication formulated with Ivabradine, which belongs to the class of hyperpolarization-activated cyclic nucleotide-gated (HCN) channel blockers. It is primarily indicated for:

  • Symptomatic treatment of chronic stable angina pectoris in adults with coronary artery disease and normal sinus rhythm who are unable to tolerate or have a contraindication to beta-blockers, or in combination with beta-blockers in patients inadequately controlled with an optimal beta-blocker dose and whose heart rate is > 60 bpm.

  • Treatment of chronic heart failure (NYHA Classes II or III) with systolic dysfunction in adult patients in sinus rhythm and with heart rate at or above 75 bpm, in combination with standard therapy including beta-blocker therapy or when beta-blocker therapy is contraindicated or not tolerated.

By selectively inhibiting the If current in the sinoatrial node, Ivabradine reduces the heart rate without affecting myocardial contractility or ventricular repolarization, leading to decreased myocardial oxygen consumption and improved cardiac efficiency.

Dosage and Administration:

  • The recommended starting dose is 5 mg twice daily. After 2 weeks of treatment, the patient’s heart rate should be determined and the dose adjusted accordingly:

    • If the heart rate is persistently above 60 bpm, the dose can be increased to 7.5 mg twice daily.

    • If the heart rate is persistently below 50 bpm or the patient experiences symptoms related to bradycardia, the dose should be decreased to 2.5 mg twice daily.

Common Side Effects:

  • Bradycardia (slow heart rate)

  • Luminous phenomena (enhanced brightness in vision)

  • Headache

  • Dizziness

  • Blurred vision

  • Atrial fibrillation

Precautions:

  • Not recommended for patients with acute decompensated heart failure, severe hypotension, or severe hepatic impairment.

  • Use with caution in patients with atrial fibrillation or other cardiac arrhythmias.

  • Avoid concomitant use with strong CYP3A4 inhibitors.

  • Not advised during pregnancy and lactation.
