Description

BISONEXT 5 mg Tablets are a prescription-only medication whose active ingredient is bisoprolol fumarate, a selective β₁-adrenergic receptor antagonist. This agent preferentially blocks β₁-receptors in cardiac tissue, leading to a decrease in heart rate, myocardial contractility, and cardiac output—thereby reducing oxygen demand and lowering blood pressure.

Key Indications

• Hypertension (High Blood Pressure): Lowers elevated blood pressure to reduce the risk of stroke, myocardial infarction, and renal complications.

• Chronic Stable Angina: Decreases frequency and severity of anginal episodes by reducing myocardial oxygen consumption.

• Chronic Heart Failure (NYHA Class II–IV): Improves left ventricular function, reduces hospitalization rates, and enhances survival when used in combination with ACE inhibitors and diuretics.

Mechanism of Action

Bisoprolol selectively antagonizes β₁-adrenergic receptors in the heart, which:

1. Slows sinoatrial (SA) node firing → lowers resting and exertional heart rate.

2. Reduces atrioventricular (AV) nodal conduction → helps stabilize certain arrhythmias.

3. Diminishes myocardial contractility → decreases cardiac workload and oxygen demand.

Dosage & Administration

• Usual Starting Dose: 5 mg once daily in the morning.

• Maintenance Dose: 5–10 mg once daily, adjusted based on blood pressure, heart rate, and tolerability (maximum 20 mg/day).

• Administration: Swallow whole with water; may be taken with or without food.

• Titration: Increase dose at intervals of at least 1–2 weeks to achieve optimal therapeutic effect.

Contraindications & Precautions

• Contraindicated: Sinus bradycardia (< 50 bpm), second- or third-degree AV block, cardiogenic shock, overt cardiac failure, sick-sinus syndrome (unless a pacemaker is in place).

• Use with Caution: Asthma or chronic obstructive pulmonary disease (COPD), severe peripheral arterial disease, diabetes mellitus (may mask hypoglycemia symptoms).

• Do Not Abruptly Discontinue: Sudden withdrawal may precipitate angina, myocardial infarction, or arrhythmias; taper dose gradually over 1–2 weeks.

Common Side Effects

• Fatigue, dizziness, headache

• Cold extremities (hands/feet)

• Gastrointestinal disturbances (nausea, diarrhea)

Patient Counseling

• Monitoring: Regularly check blood pressure and pulse; keep a log to share with your physician.

• Missed Dose: If within 12 hours of the scheduled time, take immediately; otherwise skip and resume next day—do not double up.

• Lifestyle Measures: Combine with diet, exercise, and weight reduction for optimal blood pressure control.
